Program Registration Information:

Registration Forms and Requirements

To Register you can download the appropriate form for Football or Cheerleading as well as the medical examination form. Complete the forms and bring to our office with payment to complete registration.

Registration is complete when the forms have been completed, the handbook has been signed off on, your child's school has signed off on the weekly scholastic monitoring formand payment has been made.
Inquire in the office at the time of registration about payment plans.
Registration for Football is $300

Registration fee covers:
  • Football: 2 Game Jerseys (Home and Away), Game Pants, Game Socks , Practice Jersey, Practice Pants, Combine Gear Top and Bottom, mouthpiece

  • Program Insurance

  • League Entry Fees

  • First Aid

  • Administrative Fees and Expense

  • Program Field Expense

  • Tackle Football preparation and Tackle Football Season from time of registration through November

Items parents/guardians are responsible for:

  • 7 Piece Pad Set to fit in slotted practice pants and slotted game pants

  • Cleats

  • Athletic protective cup

Football Equipment (Helmet and Shoulder Pads) are available for rental while supplies last. When supplies are no longer available for rental parents will have to purchase these items on their own. NBYSA offers parents the opportunity to purchase these items through our Equipment Representative.

Registration fee for Cheerleading is $300

Cheer Registration Fee Covers:

Cheer: A practice spirit pack(summer and fall practice uniform and cheer bag)

  • Program Insurance

  • League Entry Fees

  • First Aid

  • Administrative Cost

Additional Fees: To Be Determined at a later date

Reason: Competition Cheerleading Uniform

Includes: Top, Bottom, Spanks, Body Suit, Sneakers

Additional Fees: To Be Determined at a later date

Reason: Cheer Camp

Weekly Scholastic Monitoring

Weekly Scholastic Monitoring form to be filled out weekly for students with a grade average of 70% or lower. Form is to be filled out on Friday at school and presented at each Saturday morning practice until grade average shows improvement. Form is only acceptable when filled in by a Teacher, Signed and Stamped by a Guidance Counselor or School Official and signed by the Parent or Guardian. Students who fail to turn in the form on Saturday mornings will receive disciplinary action to be determined by NBYSA staff.
